The Office of Vital Statistics offers certified Birth Certificates for persons born in the State of Florida from 1930 to the present and Death Certificates that occurred in Seminole County from 2000 to current.

Birth Certificate

Most often, copies of birth certificates are requested for identification, to prove citizenship and for other purposes. The State of Florida maintains the original copies of birth certificates.  We provide you with a certified certificate that you can legally keep and use.

Who can obtain a copy of a birth certificate?

  • Individuals 18 and older may obtain their own certificate.
  • Parents or guardians who present court documents showing legal custody.
  • Attorneys representing children and adolescents under 18. They also must provide documentation.

To apply for a birth certificate, you must be a parent listed on the certificate, a legal representative, a legal guardian (with court appointed papers), or the individual listed on the Birth Certificate. You must be at least 18 years of age, and must present a valid state issued identification. Certificates are available for the state of Florida only. Address and phone numbers for other states may be obtained from our office or from Vital Records at CDC Web site.

Death Certificate

Death certificates are public records and anyone may obtain certified copies for a fee. If you are not an immediate family member or a legal representative, you will not be able to purchase death certificates listing the cause of death without proper authorization. Death Certificates are issued for death that occurred in Seminole County.  Fees for certified death certificates are $8.00 each.

Who can obtain a copy of a death certificate?

Without the cause of death listed:

  • Anyone may request a copy of a death certificate without the cause of death showing on the certificate.

With the cause of death listed:

  • To obtain a copy of a death certificate with the cause of death showing on the certificate, you have to be an immediate family member. This includes a spouse, brother or sister, son or daughter, grandparent or grandson or granddaughter; or
  • Beneficiary of a deceased person’s life insurance policy (must provide documentation); or
  • Estate administrator (must provide documentation).

The CDC has provided a web site for those who want direct access to individual State and territory information. To use this valuable tool, you must first determine the State or area where the birth, death, marriage, or divorce occurred, then click on that State or area.

  • Foreign Birth and Death record information.

Persons in need of a copy of a foreign birth or death record should contact the Embassy or the nearest Consulate in the U.S. of the country in which the birth or death occurred. Addresses and telephone numbers for these offices are listed in the U.S. Department of State Publication 7846, Foreign Consular offices in the United States. You may also obain your records through Vitalchek.

  • Traveling out of the country? - New travel document requirements (effective 1/2007)

Persons traveling out of the country should be aware of the new travel requirements.

  • Commemorative Birth and Marriage Certificates

certificate graphicThe State of Florida now issues commemorative birth and marriage certificates, signed by the Governor and State Registrar of Vital Statistics.

The certificates contain calligraphy style printing, gold embossed state seals, and are suitable for framing and preserving as family heirlooms. The commemorative certificates are mailed encased in cardboard shields to ensure protection.

What information is needed?

  • One (1) valid form of photo identification such as a driver’s license, State ID issued by the Department of Motor Vehicles, passport or military ID. All identification must be valid (can not be expired.) For mailed requests, please provide readable photocopies.
  • The completed Certificate Application Form.
  • You also need to provide the person’s:
  • Full name
  • Date of birth or date of death
  • County of birth
  • Mother’s full name including maiden name
  • Father’s full name
How soon may I expect a copy?
  • Your request will be processed within 3 to 5 business days from the day we receive it from you.
What is the cost to obtain a Birth or Death Certificate?
  • $12 for one copy. Additional at same time $8.00 (birth)
  • $8 for (death)
  • All requests by mail should also include the Fees and should be paid by money order or cashiers check. (Do not send cash by mail).
  • Walk-in customers may pay by cash, cashier’s check, money order or major credit card (except American Express). We are sorry, but not accept personal checks .
