Eligibility Card

• Available to all UNINSURED Seminole County Residents seeking Family Planning, Child Health or Communicable Diseases services.
• Available for Primary Care services WITHIN the Seminole County Heath Department Buildings. We do not provide Adult Primary Care, only Child Health.
• Clinic Card is NOT valid for services provided OUTSIDE the Seminole County Heath Department Buildings

• Children (Under the age of 18)
  • Family Planning
  • Communicable Diseases Control
  • Acute Care (Must have denial letter from Medicaid. Denial can NOT be for failure to follow through on eligibility process.)
  • Maternity Services
  • Well baby & Well child
  • Pharmacy & Lab relating to Family Planning, Communicable Disease, Acute Care, Well baby, Well child.
  • Medication (Epilepsy, Diabetes and Rheumatic Fever)
